REGISTRATION POLICIES

How do I register?
Complete a registration form!  These forms can be picked up at the Kingsbury Park District Office located at 114 North 2nd Street.  Once the form is filled out (be sure to include the CS#), with appropriate fee attached.  Forms can be delivered in one of 3 ways:

1.Mail to:

2. Walk-In:
Kingsbury Park District Office at
114 North 2nd Street. 

3. Drop Off:
Place it in the Drop Box located in the front of the building.  This drop box can be used 24 hours a day 7 days a week.  Please do not use cash when using this method.  

Check or money order should be made payable to the Kingsbury Park District.

Refunds:
A 100% program registration fee refund will be issued only if refund request form is received no less that 3 business days prior to the start of the program. 

A 80% program registration fee refund will be issued only if a refund request form is received prior to the 2nd meeting of the program. 

There will be no program registration refund given after the second meeting of a program except as defined under Medical Refund Request or in the case of a family moving. 

All team sports will be 50% after the draft has occurred and no refund after the first game.

Medical Refund Request
A pro rated program registration fee refund will be issued based upon the number of sessions remaining in the program only if a refund request form with a medical document.  The request must be received within three business days following the completion of the last scheduled meeting of the program. 

Program Cancellation Refund
A 100% registration refund will be issued when the Kingsbury Park District cancels a program for any reason.

Early Bird Gets the Worm!
Nothing kills a program faster than low and late registrations.  Register early to avoid program cancellation.

Shelter Refunds

Refunds will be issued only if the cancellation is made within 10 days of making the reservation.  No substitute dates will be allowed. 

Petition System for Team Sports
Parents wishing to place their child in an activity by grade rather than age and fall between the grace period of July 1st and September 1st may fill out a petition form at the Park District office.  For more information contact the office at 664-4969.
